1) Fill in the document in Chinese or English

2) All columns in the form must be complete,

pay attention to the following 5 points:

A. The “Laboratory Exam” column needs to have details, such as the testing conclusion of “AIDS, syphilis and other serological tests are …”.

B. The column “None of the following diseases of disorders found during the present examination” should be marked.

C. The column “Suggestion” should be marked with the words indicating the final conclusion of the physical examination.

D. The form should with the physician’s signature and the official stamp.

3) Please use the official physical

examination form for foreigner studying in China, and do the physical examination in formal medical institution.

5) All the medical examination results (paper

copy) are required for registration.
